The candidates shall submit the self-attested Photostat copies of the following documents with their applications:
1. MBBS Degree and State/NMC Registration Certificate.
2. MD/MS/DNB/PhD with additional Qualification Registration Certificate (State/NMC)
3. Marks Certificate of the respective degree especially Marks sheets/certificates of each year of MBBS/MSc
4. Higher Qualification Certificate (DM/MCH/DNB)
5. Any other additional qualifying certificate relevant for eligibility to any particular advertised post.
6. Experience Certificate issued by the Competent Authority/ Head of the institution.
7. Professional research publications, published in indexed/national journals.
8. Proof of writing a book (Foreign Book/National Book) published with ISBN/ISSN, if any.
9. Proof of oral paper presentation in any National/International seminar/Conference/Workshop relevant to the discipline if any.
10. Proof of poster presentation of paper(s) in a National/International seminar/ conference/ workshop relevant to the discipline if any.
11. Proof of Gold Medal for overall first position in prescribed qualification, if any.
